Bij Rondom Zwanger staan jij en jouw gezin centraal. Verloskundigenpraktijken in de regio, het verloskundig team van het Spaarne Gasthuis en kraamzorg- organisaties werken samen om jou de beste zorg te geven rondom de geboorte van je kind. Van je verloskundige als dat kan en van een gynaecoloog als dat nodig is. Zo bieden zij samen altijd de juiste zorg. Met Rondom Zwanger krijg je je persoonlijke zorgpad voor je zwangerschap.
Om onze zorg zo goed mogelijk te kunnen aanpassen aan jouw situatie, is het belangrijk dat we goed op de hoogte zijn van jouw (medische) achtergrond. Hieronder vind je vragen met betrekking tot jouw medische en sociale achtergrond. Als je dit formulier voor je eerste afspraak wilt invullen, dan neemt de verloskundige dit met jou/jullie door.
